This agreement is entered into between Union City School District ("the District") and the student and parent/guardian identified above. By accepting the device described herein, both the student and parent/guardian agree to be bound by the terms and conditions set forth below. Failure to comply may result in loss of device privileges and/or financial liability.

Charging Responsibility. The student is solely responsible for ensuring the assigned laptop is fully charged at all times. The device must be connected to a power source each night and returned to school at full charge every morning.

Daily Transport Requirement. The student is required to take the laptop home every school day for overnight charging and must bring it back to school the following day ready for use. Leaving the device at school overnight is not permitted unless specifically authorized.

Damage & Financial Liability. The student and parent/guardian accept full financial responsibility for any damage incurred while the device is in their possession. This includes but is not limited to: cracked or broken screens, liquid/water damage, broken hinges or keyboards, and loss or theft of the device. Repair or replacement costs will be billed to the family.

Care & Safe Handling. The device must be transported in a protective sleeve or bag at all times. It must never be left unattended in an unsecured location. Food, beverages, and liquids must be kept away from the device at all times.

Acceptable Use. The laptop is provided for educational purposes only. Use of the device for non-educational, illegal, or inappropriate activity is strictly prohibited and may result in immediate device revocation and disciplinary action in accordance with District policy.
By signing below, both the student and parent/guardian confirm they have read, understood, and agree to comply with all terms of this agreement. A copy of this form will be kept on file with the Union City School District Technology Department.
